First the mounting block holes are in a triangle pattern and the block can rotate a full 360 degrees so you should make sure before you start that youve rotated everything to match the template

The procedure that ended up working for me was after drilling the holes in the cabinet using your countersink if you have one remove the template from the top of the shelf then flip it and position it under the cabinet being sure to keep the front of cabinet part of the template toward the front and most importantly lining up the holes in the paper with the holes you just drilled

Once youve gotten a few turns into the mounting block youll want to rip out the template

The paper is actually pretty sturdy so if you want to be really smart before you flip the template you could cut the template from the edges to each screw hole to make it easier to rip out
