Like almost every pci card i have ever bought i needed to file down the bottom of the card to help the contacts fit in the slot better

When i first fit it in only one interface showed active as the card did not quite fit flush in the slot

A gentle rasping with a file took off the bottom of the card under the contacts enough to get the card in properly
