Additionally nowhere in the instructions did it show where those extra washers should go nor was there any logical place for them since several nuts bolts and washers came preassembled

So it came with a missing nut and two free washers

They come in the little bag with the washers cotter pins long story short they are a little piece of mettle brown in color and you need a hammer to get them in the little holes in the axle they line up once in a little grove on each side and when putting on the handle there was a piece of black plastic that was pushed in to deep that would not let the bolt go through all that was needed was to take a screwdriver and wiggle it around to push the plastic up out of the way
