The strap in the foot area is fixed (elastic but not adjustable) so you have to shove your foot through what to me seemed like kind of a small opening.

The suggested sizing on the box is true: sm/med if joint circumference is less than 9 in; lg/xlg for greater than 9.

I was just a tad over 9 w/ size 8 women's shoe but got the small/med and there's small gaps between the gel packs giving just short of 360o coverage.
