I suspect there is a lot of user error in the reviews who state they cannot hear anything through it

For basic listening use the larger round side place ear tips in ears with them slightly twisted forward tap the large round side lightly

If you only hear it faintly twist just the round part of the stethoscope and itll click into place and you should hear clearly

Cheap works and you wont execute your co workers if it goes missing

I would recommend this for the use of fun but i am not sure how reliable it would be in an actual medical setting
