In order to work correctly though the pins need to thread all the way into the handle and then then receive the knurled lock nut on the back

To prevent damage to the tool the holding pins should be loaded as close to the face of the tool as possible

The application of the load at the very end of the pins resulted in a high moment which is why the swing arm deformed twisted in this manner
