There was on one in the cap and the other 3 pieces were in the plastic bag not is the cap it came with

The problem i have is that they are very uncomfortable in your nose for something that youre wanting to be able to sleep with they are very uncomfortable even with the different sizes they offer if it werent for that they would have gotten 5 stars
