In order to get 4k 60 using this adaptor on a gtx 600 series card probably a 700 series as well you need to create a custom resolution
When making a custom 4k resolution within the nvidia control panel set the resolution to 4k and the refresh rate to 60 hz
I needed an adapter that supported 4k resolution after mistakenly purchasing a different product
