The issue is the plastic plate with the casablanca name and the fan speed settings moves slightly at the bottom where the onoff switch is located

When it moves power is lost for both the fan and the lights

Actually both switches received have the problem but the one being returned is what i consider to be a potential safety hazard

The problem was found while installing the switch plate which caused the plate to move enough to shut down both the fan and the lights

Applying slight pressure to the plastic plate with the switch out of the electrical box confirmed the problem is in the switch and not an intermittent connection in the wall electrical box for the switch
