The button on the transmitter is a simple playpause button and twisting the knob is for changing tracks for music and advancing 15 seconds on podcasts this might depend on the apps you are using

You get a very low level hiss of radio noise but this can be minimised by putting your phone volume all the way up so that you are transmitting a louder signal and the noise is proportionaly very low
