After routing out the hinges with a porter cable template i used to just try and start the screw in the center of the hinge hole but that often left the screw running off in an angle and was horrible

When they are not centered then the screw ends up on an angle when the screw head sits down into the beveled countersink opening on the hinge

Even on prehung doors that i buy from the big box stores have the hinge screws run in on angles it looks terrible

On the smallest one the pilot drill is sticking out passed the sleeve a bit
