An insert for the large * is included for 28/med just slide it inside. It's important to have a correct fit when pumping, first for comfort, 2nd to make sure you are able to get all the milk out and no ducts are blocked.

Ameda sends size small even though their website says it offers 7 different sizes. But, as other reviewers stated, the medium just sits inside the large and doesn't connect at all. This makes the mediums totally pointless.
