I wear them so the front slope is holding up the ball a little and the high point is touching the beginning of my arch. Voila, you have the perfect fit without having to unstick and reposition multiple times.
First use duct tape as a test until proper placement is satisfactory. Positioning it correctly is critical.
Cons
It has a strange shape that makes it tough to get in just the right spot to unload the pressure from the metatarsal heads. They also have almost zero actual padding. They are very thin and tore very easily.

I use them with the powerstep insert for my shoes
Not any part of the pad should be under it just like in the picture from the product container
This rather important information did not come with the pads i received nor is a visual aid included in the sales description on amazon

Would buy them again but the cost is a little steep

The pads are pretty thin but i guess if youre going to put them in your shoe they need to be but for the price i dont think its worth it

However over 6 months i realize my toes were rubbing against each other so i pulled out the custom pair and realized there was a metatarsel ridge so i bought these
