This item works very well in scraping and washing away debris.
Water-pick works better and for efficiently than a toothbrush. Works well, but if you have a bad gag reflex, it might still bother you. Improves my breath considerably.
Cons
No point to using this with a water pik. Just use a regular tongue cleaner. The scraper is too smooth.
