It is correctable but a large annoyance. Such issues with both primary functions of this product is unacceptable. This design is flawed.
The ridge in the cap that engages the o-ring isn't deep enough. There was no pop into place feel any more ensuring it was locked.
It is almost as if the threads were stripped. The only way around this is to go for a tactical pen model that screws together. It's heavier than what i am used to carrying but it's not a hotel pen either.

Of course glass shattered everywhere which is why he did it in a cardboard box

Was able to get it at a ridiculously low price thanks to jungle specials but i own several hoffman and richter items and you cannot beat their quality honest review but i did receive the technical pen at a reduced rate in order to review

When i had an issue with the pen the rubber o ring that holds the cap came loose survival life sent me a replacement at no charge
