Qc failed on this lamp. Needs to be checked in specs at assembly level. Not only is the light top heavy but mine also doesn't tighten hard enough to keep it from tilting to the side.

This seems to be a reoccurring problem with the adapters quitting or malfunctioning. There may have been a short in the adapter, causing it to get too hot and melt the adapter in places.
