To wear the belt i went with the fingers out approach where the fingers were pointing away from my body

I put on my undershirt dress shirt and then stepped into the belt working it up over the the shirt tails and around my waist again with the fingers pointing away from my body

The design also doesnt allow it to move at all once you put on your dress pants and so will never rise up above your pants
