It turned out that the load wires on the old unit were at the bottom and the line wires were the top pair

After removing the new unit and verifying which pair in my box were from the breaker box and which were to the second outlet i installed it properly and it worked

Look on your old unit to make sure you are getting the load and line wires right

The problem was not the receptacle it was the 1964 wiring and the awkward place it was located

Was not slim fit even though its advertised that way and the picture on the package displays a slim fit but its not as my pictures show
